Reporting to the Senior Clinical Manager of Family Care, the Specialty Care Navigator, Neurodiversity and Youth will primarily provide clinical navigation and family-centered support through Spring Health’s Neurodiversity Specialty Care Program. Neurodiversity support spans children and families from ages 0 to 17. The role will also support adolescents ages 13 to 17 and their caregivers within the other Specialty Care programs, including Mood and Anxiety, Trauma, Eating Disorders, and Substance Use. No travel is required.
This is a full-time, fully remote position. The primary shift is Monday–Friday, 9am-5pm PST/ 12–8 p.m. ET. Team members should be flexible to work a later shift 1–2 days per week (2–10 p.m. ET / 1–9 p.m. CT / 11 a.m.–7 p.m. PT) to support after-school scheduling and teen availability.
What you’ll do:

Neurodiversity Specialty Care:
Conduct developmentally informed and neuroaffirming telehealth screening, triage, and care navigation for children, adolescents, and caregivers seeking support with neurodiversity and related behavioral health needs, including ADHD, autism, social communication differences, and learning disorders
Provide developmentally appropriate psychoeducation and brief support to childre, teens, and caregivers navigating neurodiverse conditions
Integrate information from youth, caregivers, providers, and available clinical, developmental, neuropsychological, or school evaluations to develop a clear working formulation, identify differential considerations, and recommend appropriate next steps
Help families connect with appropriate services, including therapy, medication management, neuropsychological evaluation,, ABA, OT, SLP, and other community-based supports
Guide families through school-related supports and accommodations, including IEP and Section 504 processes, and coordinate with schools and community providers when appropriate and authorized

Teen Specialty Care:
Conduct developmentally informed telehealth screening, triage, and care navigation for adolescents 13-17yo and their caregivers across the Mood and Anxiety, Trauma, Eating Disorders, and Substance Use Disorder programs
Provide developmentally appropriate psychoeducation and brief support to adolescents and caregivers navigating mood and anxiety concerns, trauma-related symptoms, eating disorders, and substance use
Build and update individualized care plans that reflect the young person’s needs and goals, clinical presentation, level of care, family context, and available evidence; revise recommendations when new information changes the clinical picture
Help teens and families connect with appropriate services, including therapy, medication management, PRS, HLOC, and other community-based supports
Use active listening and motivational interviewing to engage adolescents and caregivers. Honor the adolescent’s voice, privacy, and developmental needs while supporting caregiver involvement and family collaboration, consistent with applicable confidentiality, consent, and safety requirements
Recognize safety concerns, provide appropriate in-the-moment support and safety planning, and coordinate timely referral to a higher level of care when indicated.
Coordinate with internal clinical and operational partners and external providers, schools, and community resources to reduce barriers and support continuity of care.
Maintain timely and accurate documentation, follow up on agreed care steps, participate in consultation and training, and contribute feedback that improves program quality and the member experience
Participate in staff meetings, case consultations, and trainings
Assist with onboarding and mentoring new team members
Follow Spring Health policies and maintain all confidentiality, compliance, and ethical standards
What success looks like:
Maintain high member satisfaction, as measured by survey responses
Triage and enroll members into appropriate specialty care program and LOC, based on presenting problems and goals
Members engage with their recommended care plan by connecting and progressing with appropriate services, which may include therapy, medication management, evaluation, Specialty Care Navigator follow-ups, occupational therapy, or speech-language services.
Consistent documentation and follow-up within required timeframes
Effective collaboration across internal and external teams and partners
What you’ll bring:
An active, independent mental health license as an LCSW, LMFT, or LPC

Neurodiversity Clinical Experience:
Minimum of 2 years of clinical experience supporting children, adolescents, caregivers, and families, including direct experience with neurodiversity such as ADHD, autism, social communication differences, or learning disorders.
Familiarity with evidence-based and family-centered supports for neurodivergent children and adolescents, including parent training, PCIT, and CBT-Family
Experience interpreting neuropsychological or school-based evaluations and incorporating findings into referral and support planning
Strong understanding of school-based supports and accommodations, including IEPs, Section 504 plans, IDEA, and coordination across school, healthcare, and community settings

Teen Clinical Experience:
Minimum of 2 years of clinical experience supporting adolescents with mood and anxiety concerns and trauma in outpatient or intensive outpatient settings with knowledge of current best practices and evidenced based treatments for this population
Working knowledge and direct experience with adolescent eating disorders and substance use presentations, family context, evidence based treatment approaches, and indicators for referral or a higher level of care
Experience with clinical assessment, diagnostic formulation, differential considerations, level-of-care recommendations, and care planning, including adjusting recommendations as new information emerges
Strong skills in motivational interviewing, psychoeducation, crisis support, safety planning, care coordination, and written and verbal communication
Ability to organize a varied caseload, document accurately, use multiple technology platforms, and adapt to changing needs in a growing program
Commitment to culturally responsive, inclusive, family-centered care that respects each young person’s strengths, identity, autonomy, and developmental needs
Motivated to go above and beyond for members to provide a high-touch experience
Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks.

Experience and training in any or all of the following evidence based models: DBT-A, TF-CBT, FBT/Maudsley Method, adolescent SUD treatment (e.g., contingency management, harm reduction, family based therapy)
Proficiency in Adolescent ASAM criteria
The target base salary range for this position is $33.33 - $42.31 Hourly, and is part of a competitive total rewards package including benefits. Individual pay may vary from the target range and is determined by a number of factors including experience, location, internal pay equity, and other relevant business considerations. We review all employee pay and compensation programs annually using Radford Global Compensation Database at minimum to ensure competitive and fair pay.
